We would like to assist you in making your transition to an established patient in our clinic as easy as possible. We have prepared these resources for you in hopes that your experience with us is smooth and satisfying.

 Your First Visit Checklist:

• Check in 15 minutes prior to your appointment time
• Obtain directions to our office prior to your appointment
   
• Bring completed New Patient Paperwork
   
• Bring Insurance Card
   
• Bring Driver’s License or Picture ID
   
• Bring a list of all medications, prescription and over-the-counter, that you are currently taking
   
• Bring any pertinent medical records that would be needed for your visit
   
• Bring Co-Pay required by your insurance
