On-line Mammogram Scheduling is now available

 

Here at Fitzgibbon Hospital, we are striving to help make life easier for our patients.  We are now offering on-line scheduling for mammograms.   You tell us when a good time is and we’ll call you back to confirm.  Please schedule your appointment post-menstrual time, unless you have an abnormality, for a kinder and more accurate reading.  Please do not schedule an appointment if you are lactating.  Click here to send an e-mail to us, or copy and paste this address into your e-mail scheduling@fitzgibbon.org, please include the following in your e-mail:

  • Patient's Name

  • Patient's Date of Birth

  • Number where you can be reached between 8:00 am and 5:00 pm

  • Date and Time that you are wanting to make your appointment

 

Be prepared to  answer the questions below when we call from our Scheduling Department to confirm the appointment.    

  • Has it been over a year since your last mammogram?

  • Who is the Doctor that ordered the mammogram?

  • Have you had a diagnosis of breast cancer in the last 5 years?

  • Have you had breast implants?

  • Are you currently having any problems? (i.e. lumps, tenderness, etc.)

  • Insurance information

  • Social Security number

If you have had a mammogram at another facility please bring a copy of the scans and all paperwork with you.
