Medical Records

If you are interested in getting a copy of your medical records, call the Health Information Management Department and they can mail or fax you a Release Form. Simply fill out the form and fax it back! Your records will be waiting for you.

If you would prefer, you can come by the Hospital, fill out the form and pick up your records.
