Diabetic Education

The mission of the Diabetic Self Management Program at Fitzgibbon Hospital is to provide quality, comprehensive diabetes self-management education for all diabetics in our community. We believe that education is the key to empowering the person with diabetes to better manage his/her disease and avoid the complications of diabetes to ultimately achieve an optimum health status.

The program at Fitzgibbon Hospital is recognized by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education. As an ADA certified facility, Fitzgibbon Hospital is eligible to bill Medicare for Diabetes Self Management services.

Diabetes Education Program will cover the disease process, types of diabetes, complications, nutrition, exercise, medications, stress, healthy habits and blood sugars. Education will also be provided on the use of a glucometer and insulin therapy if needed.

Fitzgibbon Hospital requires a referral from your primary physician in order to attend classes. Your insurance can be billed directly for these services. If you have questions regarding coverage, contact your insurance representative via phone or website. Financial assistance is available, if needed.

Additional nutritional education available for services ranging from weight management to specific disease prevention.

Hours of operation are Monday and Wednesday  from 8-4:30. Schedule can be adjusted to meet individual needs.
