Transitions Care Unit About our unit Transitions Care Unit (TCU) is a 10-bed behavioral health unit located at Fitzgibbon Hospital in Marshall. This secure inpatient unit is dedicated to serving the senior population (ages 50 and above) that may be experiencing behavioral, emotional or cognitive problems. Our dedicated team of mental health professionals include:
Shortly after admission, each professional will conduct a comprehensive assessment used by the team to formulate an individualized treatment plan that addresses each problem and establishes goals to address them. The treatment process includes individual, group and family therapies, when appropriate, along with various expressive and relaxation therapy programs. The psychiatrists regularly visit with each patient to determine the appropriate use of medications if needed. Physical needs are addressed by each patient's hospitalists and/or the TCU Medical Director, which are coordinated together as part of the treatment plan. Once the patient has met the goals of the inpatient treatment plan, a discharge plan is developed to ensure that ongoing care and well-being continues in the community. TCU has a working partnership with Royal Oaks Hospital and Pathways Community Behavioral Healthcare, Inc. who operate a number of community-based outpatient mental health programs throughout Missouri. Ongoing care with mental health professionals can be provided at one of these community clinics or nursing facilities following discharge from TCU. TCU also maintains a clinical training affiliation with the University of Missouri - Columbia, School of Medicine as an approved training program for the Department of Psychiatry, Section of Geriatric Psychiatry. This academic relationship provides our staff with the most up-to-date advances and treatment processes in geriatric medicine, ultimately enhancing the quality of patient care. Program focus Seniors often experience problems of depression or coping due to stressful issues associated with aging. This includes issues such as grief and loneliness following the death of a loved one, changes in cognitive behavior caused by stroke or other physical illness or disability, Dementia or Alzheimer's Disease, and other illnesses that impact older adults. The goal of the program is to improve each patient's ability to function more independently and enhance his or her quality of life and dignity.
