Patient Registration Admission Representatives are required to ask detailed questions in order to complete the registration process. In order to maintain accuracy with your billing statement, we must verify this information on each visit to our facility. Once the short registration process has been completed, you will be asked to sign a Consent for Treatment and Patient Agreement Form. These two forms give Fitzgibbon Hospital consent to perform the services your physician has ordered, authorizes Fitzgibbon Hospital to release information to your insurance carrier, acknowledges financial obligations and Receipt of Notice of Privacy Practice. The Admission Representative will ask for you to pay any known co-pay or deductible due on your insurance policy. Payment is expected prior to receiving services. If you do not have any insurance coverage, or have insufficient insurance coverage, payment arrangements can be made through our Patient Accounts Department at (660) 831-3700. Whether inpatient or outpatient, all patients are required to go through the registration process. Our goal is to perform the registration process as quickly and accurately as possible. In order for us to obtain our goal, please bring the following with you: All active Insurance cards Picture Identification Social Security Number Contact Person and their phone number Co-Payment/Deductible List of medications currently taking Physician referral (if needed) Physician Orders
